NEWS2 Score Calculator

Enter current vital signs to calculate the National Early Warning Score 2 (adults).

No patient identifiers. Enter vital signs only: no names, MRNs, dates of birth, or room numbers. Nothing you type is saved or sent anywhere. This is a training and reference tool; the EHR remains the record of care, and clinical concern always overrides the number.

Vital signs

Use Scale 2 only for patients with confirmed hypercapnic respiratory failure and a clinician-documented target. COPD alone is not enough.
Chronic confusion that is baseline for the patient is scored as Alert.

Response guide: 0–4 low: continue assessment. Any single parameter = 3: escalate for clinical review. 5–6 medium: urgent clinical review and escalation. 7 or more high: emergency assessment and high-level response.

Suspected infection with NEWS2 of 5 or more is a major sepsis warning signal. Notify the ED provider with the score and the abnormal components, then start the Suspected Sepsis RN protocol.
